Preoperative Considerations

Consent:

  • Pain at donor site — expected postoperative hip soreness.
  • Gait discomfort — temporary difficulty walking due to donor site sensitivity.
  • Swelling or bruising — localized to the iliac crest region
  • Infection — risk of donor site infection requiring treatment.
  • Bleeding or hematoma — may require drainage in uncommon cases.
  • Meralgia paresthetica — numbness, tingling, or burning of the anterolateral thigh from lateral femoral cutaneous nerve irritation or injury.
  • Persistent sensory changes — altered sensation around the incision, which may be temporary or (rarely) prolonged.
  • Iliac crest fracture (rare) — risk increases with larger graft harvests.
  • Scar formation — permanent incision scar, with rare hypertrophy.
  • Graft failure or resorption — inadequate integration of the harvested bone.

Anesthesia/Positioning:

  • Supine
  • Hip bump (can used rolled towel)

Other:

  • Reinforce need for early postoperative ambulation
